Our Approach
We strive to serve our clients like family. Since 1997, our Case Managers have been working with clients in Texas to improve their quality of life and increase their independence. With our on-staff Nurse Practitioner, we have the expertise to assist with medical decision-making, and we go above and beyond to be sure our clients have the services and equipment they need to be well.
Perhaps that’s why many of our clients have chosen to work with us for over 20 years.
Our Services
We go above and beyond to coordinate with community organizations to gain benefits that our participants are eligible for. This includes everything from adaptive aids and home modifications that make life easier, to representation at ARD meetings, to nursing services that help our clients thrive at home.
Our Process
We conduct an initial needs assessment interview for each participant, and provide an individualized treatment program, matching their unique needs with the services that best support them. Through regular follow-ups, we also provide flexible case management services to match their changing needs.
